For the personal Medical ID chip card, first select the card language and the autologous blood reference offered. Then enter first name, last name and date of birth. The name should match your identity document.
Suitable photo
Upload a clearly recognizable passport photo in JPG, PNG, WebP or HEIC format. The file must be no more than 10 MB in size. It's best to take a photo about one meter away; a very close-up selfie is less suitable. You don't have to crop the image yourself or remove the background – this will be done when you create it.
Important: Check your name, date of birth, language, note, and photo carefully before adding it to your cart. The card will be personalized based on these details.
